Most of the issues I have seen have been about wiring or overloading. So not tinning or bootlace crimping before sticking multi-strand wire into a connector and ending up with a few strands sticking out waiting to fry you, or just bare wires, spaghetti wiring everywhere, water around power supplies, hell people seem to think wires come in different colours to be pretty or some shit 
And nicely highly conductive aluminium frames. A safety trip switch will probably save a life in a pinch also. I’d rather not risk it
