It wouldn’t surprise me at all if that first death from cannabis (supposed thc cart) was from one of the “dispensaries” on Weedmaps or some place not even found online. Not enough information on the case to me to believe it’s exactly as claimed.

I’m out and about right now, so I’ll have to go back for references, but the black market carts causing this contain vitamin e acetate, a derivative of vitamin e oil. It’s been well known for over 10 years in the nicotine vape market that oils of any kind are a no go. Oil pneumonia is most commonly linked to Diacetyl, a butter flavoring used in popcorn for years, but can be caused by most oils inhaled for long periods of time.

Anything over a certain particle size causes issues yes. That was my first thought after I saw the news reports honestly.

I would say that happens more often than once. I have had a couple pkgs go missing and seems odd that it’s the only ones that ever have considering I get a lot more other pkgs that are other items, odds would say it shouldn’t be that. Locally there has even been a rash of community mailboxes being compromised in rural areas and the suspicion is that is what they are after.
